Objective:
Manages, implements, and coordinates the case management program in the region to ensure prescribed activities are carried out in accordance with specified objectives. Provides technical assistance and contract management in planning, development, and implementation of programs and services for the elderly in Bexar County.
Organizational Relationships:
1. Reports to: Bexar Aging Director.
2. Directs: Does not supervise any employees; however, may assign and review limited scope of work to the Bexar Aging Intake Assistant.
3. Other: Has regular contact with clients and their families, organizations of the elderly, service providers, other program personnel, area elected officials, private contractors, and the general public.
General Statement of Duties:
This is a responsible, generally non-supervisory position requiring familiarity with various government programs and insurance policies related to the elderly, and various rules and regulations regarding aging programs and services. Duties include implementing case management programs; conducting assessments; providing counseling services; tracking expenditures; coordinating services with other agencies; and providing resource information or referrals to elderly clients and/or their family members.

Essential Duties
Implements the case management program through the identification of clients, individual client assessment, individual service plan development, arrangement of necessary care and services, follow-up, and on-going monitoring of clients' status, which may include making home visits, determining eligibility, assessing hours for personal care and coordinating with contracted home health agencies, etc.;
Provides information and referral, legal services, crisis counseling, and services necessary to stabilize a situation for elderly clients and/or family members on a short-term basis;
Maintains confidential case records that may include client data and documentation of services for the client tracking system, and completes related monthly reports as required;
Provides assistance and back-up for the Information and Assistance Supervisor and Legal Awareness/Benefits Counseling Supervisor as required;
Completes benefits counseling certification and provides counseling services as an integrated service of case management;
Tracks monthly program expenditures for each budget category, such as income support, health maintenance, residential repair, and personal assistance;
Completes check request vouchers;
Attends meetings, conferences, workshops, and other in-service training programs related to the case management program and benefits counseling;
Provides periodic presentations to elected officials, community leaders, Bexar Senior Advisory Committee, and local service agencies/organizations, subcontractors, and the elderly within the region regarding case management/benefits counseling;
Develops, implements, and maintains the case management section of the policies and procedures manual;
Assists in developing the area plan and any necessary amendments by providing input based on evaluation of the case management program/benefits counseling program;
Monitors, evaluates, and provides technical assistance to subcontractors/vendors regarding service delivery to the elderly in order to assure appropriate and quality care;
Maintains current knowledge in various related government and local programs and insurance policies, including Medicare, Medicaid, other private insurances, Social Security, Social Security Insurance, disability insurance, food stamps, utility assistance pensions, money management, community-based services, and appeals processes for Medicare, Medicaid, and Social Security Insurance, etc.;
Provides assistance to senior citizens in the direct application for obtaining and maintaining insurance and entitlement benefits;
Provides or assists in advocacy and representation requiring review, waiver, reconsideration, and/or hearings before administrative agencies allowing benefits counselor advocacy and/or representation;
Builds effective collaborations and referral linkages with local legal providers, area hospitals, and other social service agencies; and
Coordinates related administrative and program activities with Information and Assistance, Ombudsman, and Case Management programs.
Other Essential Duties

Required Knowledge, Skills, and Abilities:
Ability to acquire thorough knowledge of AACOG policies and procedures and general office procedures and practices; knowledge of community resources; knowledge of AACOG procedures regarding program expenditures; ability to acquire knowledge about problems and issues regarding programs serving the aging and social policies and responses facing the state and nation; ability to analyze social systems and determine appropriate course of action; ability to work well with elderly citizens and citizen groups; ability to communicate effectively orally and in writing; ability to establish effective working relationships with other employees, clients, and work related public; ability to use personal computer, including standard word processing, spreadsheet, and data inquiry software; ability to prepare concise and accurate reports, individual service plans, and agreements; ability to handle multiple, simultaneous tasks with time constraints; and ability to work independently and creatively.
Acceptable Experience and Training:
Bachelor's degree in social work, public administration, or a related field; or licensed as a registered nurse or certified as a social worker; plus at least two years of progressively responsible experience in the field of aging, nursing, social work, other human services, or related field; (Two years of full-time aging, social work, or registered nursing experience may be substituted for one year of the required education, with the maximum substitution of four years of experience for two years of education.)

Any equivalent combination of experience and training which provides the required knowledge, skill, and abilities.
Certificates and Licenses Required:
Must complete benefits counseling certification within the first year of employment; and valid Texas driver's license or available alternate means of transportation.
