Objective:

To provide initial intake enrollment and ongoing service coordination services to consumers in gaining access to medical, social, educations, and other related needs.

 

 

Organizational Relationships:

1.      Supervisor: Mental Retardation Services Manager.

 

2.      Employees Supervised: This is a non-supervisory position.

 

3.      Other: Has regular contact with service providers, outside agencies, other program personnel, and the general public.

 

General Statement of Duties:

This is a responsible, non-supervisory position. Duties include performing intake duties to determine diagnostic and financial eligibility; coordinating services through available service providers; and monitors services being delivered.

 

Examples of Work:

Essential Duties

 

Coordinates and monitors services for consumers, including traveling to sites, performing site visits, and conducting progress note documentation;

Develops and submits care plans and other data into database;

Completes permanency plans for individuals under the age of 22 that are in an institutional setting;

Oversees that services are authorized and delivered according to guidelines and that all available community resources are utilized;

Offers program and provider choice to families and consumers;

Performs intake services to determine diagnostic and financial eligibility;

Completed enrollment packets and maintains vacancy listings; and

Attends trainings and workshops.

 

Other Important Duties

Performs other duties as required.

 

Required Knowledge, Skills, and Abilities:

Knowledge of mental retardation and Autism/PDD services and funding sources; knowledge of personnel policies and procedures; standard office practices; knowledge of casework principles and practices.  Ability to establish and maintain effective working relationships with individuals from all socioeconomic levels; ability to communicate effectively orally and in writing; and ability to operate basic software, including word processing, spreadsheet, databases, and presentation software.

Acceptable Experience and Training:

A bachelor's degree in behavioral science, plus one year of mental retardation experience. 

OR

 

Any equivalent combination of experience and training which provides the required knowledge, skills, and abilities.

 

Certificates and Licenses Required:

Valid Texas driver's license or available alternate means of transportation.
